Council no longer issues School Entry Immunisation Certificates.
An immunisation certificate for school enrolment purposes can only be issued on completion of all scheduled infant immunisations up to and including the four year old immunisations.
After you child has completed the four year old scheduled vaccines, the Australian Childhood Immunisation Register (ACIR) will automatically send you a history statement. This statement should state:
- "Child has received all vaccines required by five years of age".
This statement now acts the School Entry Certificate for school enrolment.
