How to apply for assistance

You can apply for a service. A relative, friend, nurse, general practitioner or another service can also apply on your behalf.

Are you 65 or older?

If you are 65 years or older you will need to register with My Aged Care. My Aged Care is a federal government initiative that is designed to give people more choice, more control and easier access to a full range of aged care services.

My Aged Care will send us a referral for an assessment and a member of our assessment team will then be in touch with you to arrange a home support assessment.

What is a home support assessment?

A home support assessment aims to understand your needs, and work out the care and services that will best support you to remain living at home safely. You are welcome to have someone with you, such as a family member or friend, during this visit.

During the assessment, a support plan to meet your individual needs is developed. The plan may also include referrals to a range of other professional services as needed, such as physiotherapy, occupational therapy or podiatry.

There is no obligation to accept services and there is no charge for an assessment visit.
