Access aged care services with Care Finder

Our Care Finder program can help you to learn about, apply for and set up support services

About Care Finder

Care Finder is a free program provided by experienced and friendly Care Finder staff. It exists to support vulnerable older people – who have no one else who can support them – to learn about, apply for and set up support services.

The Care Finder program is funded by the Australian Government.

Care Finders can help with:

  • registering you with My Aged Care
  • finding the right services and connecting you to support such as assistance at home, health, mental health, and housing to improve your quality of life
  • supporting you while services are being organised.

    Bayside Council’s Care Finders are skilled listeners who provide ongoing face-to-face support while services/supports are being set up and then checking in once services have commenced.

Eligibility

To be eligible for the Care Finder program through Bayside City Council, you are aged 65 or over, 50 and over if Aboriginal or a Torres Strait Islander or 45 and over if you are homeless or at risk of homelessness, and you:

  • are living in Bayside
  • are unable to access aged care services on your own
  • do not have family or others to assist you to access services.

How to access the Care Finder Program

Council’s Care Finder program accepts referrals from health and community services, family and friends as well as self-referrals or from other sources.
