DIY book club program application form

The Bayside Library Service book packs are available to all members of the Library Service on payment of an annual subscription fee of $150 (including GST).

Members will be entitled to:

  • Unlimited loans from the book pack collection each year
  • Six-week loan period for book packs
  • 12 copies of each title in pack
  • Background notes and discussion questions
  • Collection and return from any branch

Regulations:

  • One club member (coordinator) must belong to Bayside Library Service and take full responsibility for all items borrowed on their card. 
  • That all 12 books are returned together on or before the due date and in good condition.
