Care Coordination

For people needing a higher level of care, Bayside Care and Support offers Care Coordination through a Commonwealth Home Care Package. Once you are matched with one of our expert Care Coordinators, they will work with you to manage your care budget and connect you with services. Our Care Coordinators can also organise specialist services including physiotherapy, podiatry, specialist nursing care, transport services, and social connections.

The type of assistance provided depends on the level of care you’ve been assessed for by the Commonwealth Government’s My Aged Care team.
