Youth Services School Holiday activities - terms and conditions

Young People must be aged 12 to 17 years old when participating in activities.

Bookings

  • Are open to young people who live, go to school, work or study in the Bayside City Council area.
  • All bookings are to be made online at www.bayside.vic.gov.au/SHA
  • All bookings are considered final once made and are non-transferable, and non-refundable. 

How to Pay

  • Fees will be deducted from your credit or debit card.
  • A 50% discount is available to Bayside young people named on a health care card or equivalent. Please contact Youth Services with these details prior to registering: youth_services@bayside.vic.gov.au, or 9599 4622.

Important information

  • All workers are qualified and experienced.
  • At least one worker on each activity has First Aid and anaphylaxis qualifications. First Aid kits will be carried at all times. 
  • There will be a minimum of one staff member to every ten participants.
  • Staff are not responsible for young people outside the advertised activity time or if they are signed out.
  • Parents/carers/guardians can choose to allow their young person to sign out and leave independently, please see registration form. 
  • Please contact the lead worker if you are running late for an activity or can no longer make it.
  • The program operates on a warning system for unacceptable behaviour. A maximum of three warnings can be given before the participant’s enrolment will be cancelled and no refund will be given. Parents will be notified of all warnings issued.
  • Participants are responsible for their own belongings. Bayside City Council will not be held responsible for the loss or misuse of property or spending money.
  • To cater for all weather conditions, we ask all young people wear appropriate clothing, including hats, sunscreen, long sleeve tops, raincoats and carry water bottles.

Medical and Property Liability

Parent/guardian permission declaration: I, as parent/carer, agree that Bayside City Council, its officers or servants will in no way be held liable for the loss of property or any damage or injury howsoever caused or of whatsoever nature that may be incurred by my young person in attendance in conjunction with the said program. I acknowledge that while every reasonable effort to minimise exposure to known risk will be taken, all hazards and known dangers associated with these activities cannot be foreseen or may be beyond the control of the organising body. I acknowledge that I have been provided with all the information I require regarding the program as outlined, and/or may contact Bayside Youth Services youth_services@bayside.vic.gov.au or 03 9599 4622 (during business hours) should I require clarity regarding the program. In the event of accident or illness, I authorise the staff member(s) in charge consent, where it is impracticable to communicate with me or emergency contact person, to my child receiving medical or surgical treatment as may be deemed necessary, transportation and/or hospitalisation and agree to pay for fees and charges incurred. I understand in the event of personal accident and/or property damage I am not covered under the auspices of the Bayside City Council insurance program, unless it is proven to be a result of negligence on part of the Council or its employees.

Refund Policy

  • Refunds will only be given in the event of illness/emergency with a medical certificate.
  • No alterations or transfers will be accepted as all payment is final at the time of booking. 
  • Full refunds will be given if the activity is cancelled for reasons out of our control.

Bayside School Holiday Activities aim to provide an inclusive service for young people. If your young person requires support to participate in school holiday activities, please contact youth services on 9599 4622 or email youth_services@bayside.vic.gov.au prior to booking into the program.

Privacy Statement

Council is collecting the personal information on this form for the Bayside School Holiday Activities. The personal information will be used solely by the Council for the purpose of program registration or directly related purposes such as an emergency. The applicant understands that the personal information provided is for the above-mentioned purpose and that he or she may apply to Council for the access to and/or amend the information recorded. Requests of access and/or correction can be made to Council’s Privacy Officer.
